Medical Social Work

Programs that provide support services for patients and their families during hospitalization and upon discharge, for people receiving outpatient services, for previous patients and for other people not previously associated with the facility who need the service. Included are consultation and the coordination of available services for the patient's continuing care at home or in a short or long-term care facility; or whatever other support may be needed to help resolve the logistical, social and psychological problems related to the illness.

Hospice Care

Programs that provide a full range of supportive services for terminally ill individuals who are in the final stages of their illnesses and for their families. Services may include medical care, pain and symptom management, home nurse visitation, case management, emotional and spiritual support, and bereavement services for the patient and members of the family. Hospice care may be provided at home, in a freestanding hospice facility, a hospice unit of a hospital or in long-term care facilities such as nursing homes.

Bereavement and Grief Support Groups

Mutual support groups whose members are people who have experienced a loss such as the death of a loved one, divorce or separation and are trying to cope with their grief as a result. The groups meet in-person, by telephone or via the Internet; and focus on helping participants accept their loss, express their grief, move through the bereavement process and put their lives back together.

Counselling programs or other organizations that are actively seeking individuals with the requisite skills and experience who are willing to work on a voluntary basis without remuneration providing emotional support, problem-solving assistance, information and guidance for people who have recently lost a loved one.

Faith Based Counselling

Programs that specialize in providing therapeutic interventions which focus on helping people relate to and resolve their problems and concerns in the context of their religious/spiritual beliefs. Services are typically provided by ministers, rabbis, priests, imams or lay representatives of a religious tradition who integrate modern psychological thought and methods with traditional religious training. Included are pastoral counselling programs which are offered in a variety of settings including pastoral counselling centres, inpatient and outpatient mental health facilities and in private practice; chaplaincy services where non-denominational clerics are attached to a secular institution such as a hospital, prison, military unit, school, business, police department, fire department, university or private chapel; and other religious counselling disciplines.

Programs that provide emotional support, problem-solving assistance, information and guidance for people who have recently experienced a loss, such as the death of a loved one, divorce or separation.
